Details of where to send completed forms to can be found in the individual application forms. Under RA 1440, If you are applying to attend the Flying supervisors course (FSC) you will need to provide evidence of completion of the Flying authorisers course (FLAC), or if you have not attended the FLAC you will need to apply for a waiver using MAA 03, Annex B prior to applying for the course.
Use the MAA course application form for the following courses:
- Duty holders air safety course (DHASC)
- Flying authorisers course (FLAC)
- Flying supervisors course (FSC)
- Flight safety officers course (FSOC)
- Post crash management incident officers course (PCMIOC)
